Guard Williams was employed at Stony Mountain Penitentiary, Manitoba at the time of his death. He died as a result of an explosion during construction at the facility. Mr. Williams was in charge of a group of inmates engaged in Blasting rock in preparation for additions to the institution.

One charge did not go off, so after reasonable amount of time he approached the live charge, when it detached He caught the full blast on his head shatering his skull. Due to the circumstances a coroners inquest was deemed unnecessary.
