Constable Thomas Carroll was stationed at Churchill, Manitoba. He died as the result of an Airplane Crash at Cyril Lake, Manitoba. Also killed in the crash were the pilot Alex Lazaruk and Indian agent Donald McEwan. They were on a three day routine patrol at the time of the incident.
