Sergeant O'Malley drowned in Gillam, Manitoba. At the time of his death he was inspecting a boom stretched across the Kettle River that had been constructed to assist in the search of a man who had drowned in the area earlier. While clearing the boom of debris he fell in and the swift water made his escape impossible.

His remains were not recovered until 5 years after his death.
